The Embassy orders a coordination number for children of Swedish citizens who have never held a Swedish passport, national ID card, or been registered as residents in Sweden, before a passport application can be submitted. If a name notification has not been submitted by the guardians, this will be done at the same time.
The Embassy orders coordination numbers from the Swedish Tax Agency (Skatteverket) only for passport purposes. A passport application must therefore be submitted as soon as possible after the coordination number has been issued by the Swedish Tax Agency.
The Swedish Tax Agency form “First Name and Surname – Application” (Form SKV 7750) must be filled in and signed by both guardians (unless one guardian has sole custody, which must then be proven).
The child must come to the Embassy together with at least one guardian.
Original birth certificate in English.
Original or certified copies of both guardians passports.*
The guardians marriage certificate translated into English with an apostille, if they are/were married.**
Discharge letter from the hospital translated into English with an apostille.
The Swedish guardian must also prove their citizenship by presenting:
If the Swedish guardian is solely a Swedish citizen: Israeli visa/residence permit or a certificate from the Israeli Ministry of Interior regarding status in the country if you are residing in Israel (“Confirmation of non-acquisition of Israeli citizenship”).
If the Swedish guardian has dual citizenship and was born in Sweden: The certificate “Certificate Attesting Israeli Citizenship” from the Israeli Ministry of Interior. Note that the certificate must include, among other things, the date (day, month, and year) when the parent acquired Israeli citizenship, as well as the relevant section of the Israeli Citizenship Law under which it was obtained.
If the Swedish guardian has dual citizenship and was born in Israel: The certificate “Retention of Swedish citizenship after the age of 22” if the person has never been domiciled in Sweden and has not visited Sweden regularly under circumstances that demonstrate a connection to the country.

Documents in languages other than Swedish or English must be translated by an authorised translator and be provided with an apostille.
* If the child has only one legal guardian, this must be supported by a certificate or a legally binding court decision.
** Children born abroad before 1 April 2015 to an unmarried Swedish father and a foreign mother did not acquire Swedish citizenship at birth. The Swedish father may submit a notification of Swedish citizenship for the child to the Swedish Migration Agency.

New appointments are published on the 15th every three months, namely on 15 February, May, August, and November. If the 15th falls on a weekend or public holiday, appointments are published on the next working day. Due to staff changes, available appointment slots will be published one month at a time during the spring.
If, at the time of application, it becomes apparent that the required forms have not been completed or the requested certificates cannot be presented, the embassy will not be able to process the case. In such instances, you may need to book a new appointment when they become available.
