Download, print and fill in the form "Information for verification of Swedish citizenship".
If the child is resident and registered in the Netherlands, bring an extract from the population registration (Basisregistratie Personen (BRP)), from the Dutch municipality where the child is registered. This extract should state the Swedish citizenship of the child. The document must be original and no older than one month. Alternatively, a Dutch residence permit (verblijfsdocument) may be presented.
If the child is resident in a country other than Sweden or the Netherlands, a certificate of registration with information on Swedish citizenship must be presented from the country's authority, or a valid residence permit from the country. The certificate may not be older than one month.
If the child has dual citizenship (or more), show a proof of the other citizenship(s), for instance a passport.
Download, print and fill in the Guardian Certificate. It has to be signed by both guardians and witnessed by two persons.
If the child only has one guardian we need to see legally valid proof (extract from Swedish population register/court order).
We need to see the passports/valid photo ID from both parents/guardians. If only one guardian accompanies the child he/she needs to bring the absent guardian’s valid passport/photo ID (original document - no copy!).
